Self Payment for the Lap Band ® System

Self payment for placement of a band is $16,000, the price of a small car. A portion of this fee may be eligible for tax deduction. To learn more about deduction of medical expenses visit www.irs.gov or call 1-800-829-1040. Financing options are available by calling 1-800-LAP BAND and selecting option 3.

Complete payment is required at least one week prior to surgery. Payment of this fee covers the following services:

Before Surgery	The Day of Surgery	Aftercare
Surgical consultation	Preadmission testing	1 yr follow up appointments with your surgeon
Esophagram & radiologist interpretation fee	Anesthesia and Surgeon fees	Band adjustments for one year
EKG & Interpretations	Operating and recovery room services
Standard Laboratory Tests	The band appliance and supplies
Psychological Evaluation	23-hour observation hospital stay

Exclusions

The following is a limited list of what is NOT included in the self-pay fee, for which you may incur additional charges and for which you will be responsible:

• Specialist consultation(s) prior to surgery (e.g., cardiology or pulmonary) or other consulting physician fees (e.g., pathology).
• Additional testing ordered by the surgeon or other consulting physicians, e.g., a stress test, pulmonary function test, colonoscopy or laboratory tests.
• Although we do not anticipate complications, additional operative time, extended hospital stay, readmission or other associated costs are also your financial responsibility.
